I'm not aware of any problems that would have caused mail to be lost.
There have definitely been situations that would have caused mail to be
delayed, but none that would cause a loss of mail.  Properly configured
mailers should attempt to deliver mail for at least 3 days.  Generally,
we'd have to have an outage that lasted for more than 3 days for mail to be
bounced.
