I like  Diana Larsen Freeman's "Grammar Dimensions". It has those "discover the grammar" activities, problem solution activities and clear charts and tables, and it has some very nice TOEFL exercises in the Workbook.

I teach in a CUNY in New York City for ESL students.  I love all of Betty Azar's Grammar books as well as Diane Larsen Freeman's.  I have also found that Susan Kesner Bland's Grammar Sense provides a simpler combination of Azar and Freeman.  What I like of all three authors is that the present grammar in the following format:  form, meaning, and function.  Grammar is integrated in all communication skills and not just taught as an isolated topic.
