I forgot to mention it earlier, but my source for where quote marks go with reference to other punctuation is United States Govt Printing Office's Style Manual (which I last looked at about 40 years ago), but from observation, all the major American book publishers seem to follow it, too. I think the British practice was too much trouble for the American brain.
