This recommendation wouldn't cover all your needs, but Don Killgallon's Grammar for Middle School would be a very useful text for pre-service teachers.  At the end of the course, students can write sentences using prepositional phrases, infinitive phrases, gerund phrases, past and present participial phrases, absolute phrases, opening and delayed adjectives, opening and delayed adverbs, adjective clauses, adverb clauses, and noun clauses.  It is easy and fun to use, effectively teaches students to understand grammar, and has the advantage of being designed for twelve-year-olds, so it goes sufficiently slowly to be useful for adults.
