You are in luck. Right here in Seattle at North Seattle Community College there are two of us who use Sentence Combining with the teaching of grammar and we have written a book that we use. You can call Edith at 528-4547 or Michael at 528.4540 to talk with us about getting the book. McCleary is right about their being a heavy focus on how the grammar relates to style in this approach--rhetorical grammar. We have found that it helps students become better writers and readers--at least they say it helps their reading.
The Writer's Options is another text that you could look at for a related approach.

Subject: SENTENCE COMBINING
I am a student teacher from the Universtiy of Washington and am teaching a
9th grade English class in the fall. I have recently begun compiling
ideas for lesson planning and was wondering if anyone out there has had
any experience teaching grammar using sentence combining. If so, was
there anything that helped make this endeavor successful/unsuccessful?
Do you have any alternative ideas that could help me to make grammar
