At Case Western Reserve University I use the Factory Mutual System Hot Work
Permit form(yellow 6" by 8 1/2"). Our program is based on FM guidelines
modified to accomodate  the somewhat unique situations often encountered at
a University.

This plan is to be followed by outside contractors as well as our own
maintenance people.Training is provided as required.
