The trick with my sentence is one that doesn't work well in writing.
Cataphora works when the clause containing the pronoun is presupposed to
be true and the clause containing the referent is asserted. We normally
mark this relating with a subordinating conjunction, since we rather
like to put presupposed material into subordinate clauses. However, the
pragmatic relationship of the clauses can also be marked intonationally,
as in the sentence I provided. I've created a crude intonation contour
above it.

- - - - - - - - - - -
- -
He ate five green apples, and Sammy got an upset stomach.
Notice that I'm marking only pitch and not accent. With this contour,
and the pragmatics it encodes, the cataphoric interpretation works.

I do; the pronoun needs to be in a subordinate clause in order to refer
forward.

How many other listers have trouble understanding "he" as referring to
"Sammy" in this sentence from Herb's post:
He ate five green apples, and Sammy got an upset stomach.
