I hate to say this about 4.1.3, but since we upgraded 4.1.2 to 4.1.3 of our 
CAM/CAS's, we have been seeing this error.

"Unable to add user to Clean Access Server"

This is displayed to a user via the web logon and CCA Agent logon. When 
they try to authenticate to the network they are given this message and 
they're never logged into the network, so they'll try to authenticate again 
creating a vicious cycle. Eventually the user is logged into the network, 
but it takes 2,4 or even 6 tries.

We are layer 3 in-band, running Agent version 4.1.3.2. We have only seen 
this on Windows machines, so I'm not sure if this happening on MACs.

My question is has anyone seen this before and how did you fix it?
