I have been seeing the same thing but I can't get a machine to do it
consistently. I have one on my desk that would do the "get stuck in
temp role thing" but now it works fine. I keep hoping I can get it to
do it again so I can open a tac case

Login Failed Means exactly that.
The client failed to login.
Either the username / password is invalid The CCAM cannot talk to the
User/Pass datastore. (Active Directory, LDAP, etc)
Use the Auth Test to further diagnose this
