At Calif. State University, Long Beach we do not require a summary of
the teaching evaluations for candidates who are interviewing for a job
but we do require a summary for retention, tenure, and promotion
evaluations.  We require a three year summary and the format that we use
is to have the candidates list the questions from the student evaluation
form vertically and then the semester and year designations are lined up
horizontally at the top and the scores are then inserted into the
matrix.  That format is a good summary structure.  Good luck.
