There is additional information on the server-side includes supported by
MiaVX1 at: http://www.er6.eng.ohio-state.edu/www/doc/preproc.htmlx.  You
are also correct that the file must have a suffix of .htmlx (or .htx)
before it will be processed by the pre-processor.
