let me suggest two online survey services

Surveymonkey and Zoomerang.

I prefer Zoomerang as it has been around a while longer and it has
numerous prebuilt templates that one can modify to your own purpose.
Also they have a special subscription rate for educational
institutions and non-profit organization.
