Hi all,

Many, many moons ago--1999 to be precise--someone from the listserve 
sent me a boilerplate collection policy used in New York 
museums/archives, etc.  A colleague asked me for this recently and I'm 
sure it hit the circular file long ago because I can't find it--nor the 
old policy I had based on that layout.  It had an awesome layout; does 
anyone have that boilerplate form?
