There are no exceptions (but some duplications).  An is not used before a
vowel but a vowel sound: a united group; an untied group.  A herald; a
heraldic device/an heraldic device [silent h]; likewise, a is used before a
consonant sound. a history; a historical book/an historical book.
If the a is stressed, "It's A historical explanation not THE historical
explanation." [capitalization represents stressed syllable in speech].
